    Question Form Processing- redirect

    Hi I have been trying to add a contact form to a clients page. I was getting the http 404 error. Thanks to one of the other responses, I corrected that. My problem is that the redirect isnt working as well. I used a free script that came with his hosting package and the confirmation isnt going to the one I created, but the default they created if you dont select one. please advise. attached is my html for this site.
