Help with asp form

Hi there,
I'm having a problem formatting an .asp contact form. I'm confused about what I need. I have one that was cut and pasted (I Know, I know), that I tried to manipulate to ask what I wanted to ask, but I don't know enough about it to do it correctly. Here is the form, if anyone would be willing to look at it and HELP!!! I'd really appreciate it.
