how to create a 5 point Javascript rating Slider for online survey form:

I need to create a slider like this in JS/JQuery - http://www.filamentgroup.com/examples/slider/ for survey rating questionnaire.
please help me with the code for doing the similar thing with the JS/JQuery.
Need to complete this urgently.

One more sample is here - Javascript Sliders - http://www.pagecolumn.com/webparts/sliders.htm but it uses the solid colors for sliding and 1-100 rating scale n i have to use the custom image and 5 point rating options not 1 to 100 scale like in first example.

I knw there is a slider plugin also in JQuery but how to customize it for a 5 rating scale just in case used. Pls respond.
