Rating System via survey

Here is what I'm trying to accomplish:

1) Targeted surveys are sent to specific people (logins required). Select who you dealt with & Rate satisfaction (1-5)
2) Average score for each person is to be posted to a website and sorted by location.

There are several categories, so there is much more data, but this is the general idea. Non-tech savvy client wants to upload the data.

Survey software can get me the data in excel which can be manipulated, but leaves a tedious task of compiling and uploading data. is there a easier (and prettier) way than an iframe and save as html with excel? perhaps some canned software or scripts?
